Please note that although we are able to receive referral information from your GP by fax or online, we do require that you personally phone our reception in order to organise an appointment time and to ensure that Dr Mison is able to see you for your particular condition.
For your consultation please bring with you:
- A valid referral and health Summary from your GP
- Current Medicare card or DVA card
- Current Health Fund Details
- X-rays or scans related to your condition
- The consultation fee
Workers Compensation and 3rd Party Consultations
You will require:
- Prior approval in writing for your consultation payment (Obtained from the Insurance Company or workplace).
- Claim identification number.
- Full contact details of your employer or Insurance Company.
Failure to provide this necessary documentation necessitates full payment of the account on the day by the patient.
